The CDPH Gateway/CAIR2 IZ Portal facilitates electronic immunization data exchange for providers located in 50 counties in the state of California.

Important Note: Providers located in the 8 counties below cannot enroll here.

San Diego County Locations Please Read:

For those of you located in San Diego County, when you enroll in the CAIR IZ Portal you will receive a pop-up message stating that the city you are trying to enroll is not participating. If you see this pop-up window, click the “OK” button and you will be able to resume your enrollment.

If you are currently enrolled or are unsure if your site is enrolled with CAIR please reach out to CAIRDXEnrollments@cdph.ca.gov and provide the following information so we can provide you with the next steps:

• Site Name
• Site Address
• Name of Electronic Health Record software
• Address change

Before enrolling, please review the Data Exchange FAQS

When enrolling, please allow at least 15-20 minutes and prepare to have the below information ready:

1. NPI Number NPI Lookup - Search the Current NPI Registry (Apr 2026)
2. EHR Vendor Support Contact or Internal IT Contact that will support data exchange for your practice:
• Contact Name
• Contact Email Address
• Contact Phone Number
3. Choose your data exchange site type (Consult with your EHR vendor / Review the CAIR DX Enrollment guide):
   • Option #1: Direct submission (EHR → CAIR)
   • Option #2: Indirect submission (EHR → 3rd Party Submitter → CAIR)
   • Option #3: Submit data to CAIR on behalf of other Sites

After enrollment, you will receive follow-up emails with further instructions.
