CAIR Data Exchange Enrollment

CAIR Data Exchange enrollment allows providers located in the state of California to securely send immunization data from an Electronic Health Record (EHR) to the California Immunization Registry (CAIR).

Not Eligible for This Portal

If your practice is in one of these counties, do not enroll here:

Alpine, Amador, Calaveras, Mariposa, Merced, San Joaquin, Stanislaus, Tuolumne

Include:

  • Site name
  • Site address
  • EHR system name
  • Any recent address changes

Set aside 15–20 minutes and review the Data Exchange FAQs.

Have this information ready:

  1. NPI Number
  2. EHR / IT Contact
    • Name
    • Email
    • Phone number
  3. Data Exchange Type (confirm with your EHR vendor)
    • Option #1: Direct submission (EHR → CAIR)
    • Option #2: Indirect submission (EHR → 3rd Party Submitter → CAIR)
    • Option #3: Submit data to CAIR on behalf of other Sites
